Idiopathic orbital inflammation, previously known as orbital inflammatory pseudotumor is a nonspesific idiopathic inflammatory process that can involve any tissue of the orbit. Painful exofthalmos and good response to corticosteroid therapy are the hallmarks of the disease. The incidence of orbital pseudotumor have been reported as 8.5% of all intraorbital mass lesions. We present a case with meningeal signs( suggestive of intradural extension) and bilateral involvement which are extremely rare complications of the disease.
